After residency in Pediatrics at UNM, he joined the second class of the National Health Service Corps and served for 5 years as a general medical officer in Trinidad, California at the United Indian Health Services Clinic. During that time he made a decision to practice as a family physician and used the route of the "grandfather clause" to became Board Certified by the American Board of Family Practice in 1976. He has recertified several times since then.

In 1978, he joined the faculty of the Department of Family, Community and Emergency Medicine at The University of New Mexico as an Assistant Professor and Medical Director of the main teaching clinic. In 1982 he became the Residency Program Director, a post he held until 1997. During that time, the UNM Family Practice Residency Program became nationally ranked, as did the UNM School of Medicine for the innovative medical education methods used. Dr. Umland was very involved in the administration of the medical education programs and in faculty development, helping teach the problem-based-learning (PBL) methodology to faculty in the U.S. and abroad. In 1995 he spent two months at the Suez Canal University assisting with the General Practice Residency, that had been recently established there. In 1990-91 he spent six months at Xian Medical University in Xian, China, helping to develop community-oriented PBL teaching in their rural medical school branches.

His publications have focused primarily on medical education topics, as have his presentations at nation meetings. Although he retired from fulltime teaching in 1999, he remains active in faculty development and teaching the problem-based format. He joined the Advisory Board of the Institute for International Medical Education in 2000.
